JOB DEFINITION: Work with the CN Guidance to deliver and link services to the identified communities. Provide services focused on enhancing continuity of care. EDUCATIONAL REQUIREMENTS: Licensed Masters Social Work (LMSW) or Licensed Mental Health Counselors (LMHC) required. EXPERIENCE REQUIRED: Prior successful field work, community work, clinical work experience under proper supervision. Familiarly with the resources and services in the Afghan community. DUTIES AND RESPONSIBILITIES: Provide culturally attuned outreach intended to engage community. Build community trust within identified community. Support health-literacy education. Outreach and engage community members and leaders. Build a strong referral pathway of Afghan new arrivals into behavioral healthcare. Facilitate wellness groups. Provide clinical counseling to individuals and families in person and virtually. Link clients to necessary community resources. In-person presence at community hubs highly attended by Afghan new arrivals, including Community events (e.g., Diwali, Vaisakhi, other cultural or spiritual festivals) Documented learnings specific to this target population that can be used continually in future CN Guidance staff trainings; and increased cultural competency and nuanced attunement of our agency across Afghan groups long after the grant ends. To be out in the field, in the Afghan community, building awareness about CN Guidance's mental health, cooccurring substance use treatment, and wrap-around service programs. Implements appropriate crisis services when needed to prevent decomposition and move client toward an appropriate treatment to enhance adaptive functioning. Shares information about the Agency and its services in order to help clients and other interested parties participate in treatment decisions, set appropriate goals and better understand the functions of the center. Helps clients to become effective consumers of mental health and substance abuse services through information sharing, empowerment and transcendence of the stigma associated with those services. Maintains client stability through a planned ongoing treatment relationship or assesses and helps implement changes in client functioning when indicated. Regularly assesses in weekly supervision with the supervisor the progress of the client's treatment and is open to recommendations for improvement. Assists clients in developing appropriate expectations from treatment. Establishes and maintains appropriate and timely clinical case records. Exchanging information with other agencies that benefits the client, or ensures compliance with regulations and coordination of services. Participates in all required internal and external meetings that appropriately represent the needs of the client and the Agency. Utilizes and understands the value of supervision which increases practice skills, promotes better service for the client and preserves accountability to the Agency. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress Complete QI Training during onboarding and participate in QI refresher training as needed.
