About The Position

Family First Health is currently seeking a Full-time Licensed Mental Health Professional who is passionate about supporting patients across the lifespan—from age 5 through 100. This position will serve our York and our Lewisberry locations and offers the opportunity to make a real difference in the lives of patients through integrated, team-based care. We are seeking a candidate who is comfortable and experienced working with children ages 5–11, as this population represents a key component of the services provided. Position Overview: As a Licensed Mental Health Professional, you will work collaboratively as part of a multidisciplinary Primary Care team to address the mental and behavioral health needs of patients. Your role will include: Providing brief (15–30 minute), real-time behavioral health interventions for primary care patients (ages 5 and up) Conducting assessments, evaluations, and short-term treatment as needed Focusing on pediatric patients, especially ages 5–11, for behavioral health support Coordinating referrals to specialty outpatient mental health providers when necessary Supporting the integration of behavioral health into the primary care setting

Requirements

  • Licensed in the state of Pennsylvania as an LCSW, LPC, LMFT, or similar credential
  • Experience working in a primary care, community health, or integrated care setting preferred
  • Demonstrated comfort and clinical experience working with children, especially ages 5–11
  • Excellent communication and collaboration skills
  • Passion for working in underserved communities
  • Active Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Marriage and Family Therapist (LMFT) in the state of Pennsylvania.
  • Current CPR and Narcan certification
  • 2 years of clinical experience
  • Experience working with Electronic Health Records (EHRs)
  • Knowledge of Trauma Informed Care, Motivational Interviewing, Recovery and Illness Management practices
  • Knowledge of computer software applications (Microsoft Office Applications, others)
  • Knowledge of grammar, spelling, and punctuation as it relates to correspondence necessary to perform essential job functions
  • Excellent customer service skills
  • Ability to critically think which means the ability to think clearly and rationally about what to do or what to believe; understand logical connections between ideas; and identify, construct, and evaluate arguments
  • Ability to set positive examples for team members through exceptional work performance including adherence to company policies and processes.
  • Ability to work independently and within a multidisciplinary team

  • Ability to adhere to all Family First Health’s policies, processes, and procedures, especially, Code of Conduct, Workplace Conduct, and the Remote/Telework Policy if indicated eligible to work remotely or telework.

Responsibilities

  • Provides targeted evidence-based assessments and evaluations for behavioral health issues identified within the Primary Care Setting.
  • Utilizes the G.A.T.H.E.R model of care
  • Provides brief, point of care, solution focused interventions to Primary care clients to promote well-being and assist with implementation of treatment plans
  • Provides skill building, coaching, and psychoeducation to enhance whole health
  • Provides assessments to determine appropriate level of care and provide that feedback and recommendations to Primary Care team.
  • Provides consultation to patients, Primary Care Providers and other members of the patient care team in a fast-paced environment. Consultations can be both formal and informal.
  • Utilize screening and measurement tools to monitor patients response to care
  • Provide advocacy and case management services for clients
  • Maintains knowledge of community resources and coordinate refers out as appropriate
  • Document and maintain electronic health record (EHR) and ancillary records in a manner that assures compliance with all agency policies, program procedures and local, state, and federal regulations. Maintain accurate and up-to-date documentation as required, associated with billing, assessment, treatment, client’s progress and other activities.
  • Develop a comprehensive individualized treatment plan collaboratively with patients and Primary Care Provider to address biological, psychological and social aspects of wellness.
  • Assist in crisis- situations as appropriate, following emergency protocols/procedures and coordinating with the treatment team with interventions, which support a trauma informed environment for clients and staff.
  • Provide crisis intervention services to patients in order to minimize or eliminate periods of crisis and acute care utilization.
  • Requires some travel between offices for various meetings and potential occasional evening work
  • Demonstrates an ability to be flexible in workspace, meeting with patients in exam rooms as needed and station oneself in the clinical POD area.
  • Meets performance standards for productivity, documentation and adherence to metrics
  • Adhere to compliance, required laws and trainings, safety of property and staff, regulations,
  • Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings.
  • Complete all annual employee training requirements in order to understand how to conduct ethical work, for professional development, and stay in compliance
  • Perform other duties as assigned
