About The Position

The Licensed Mental Health Counselor (LMHC) position involves conducting clinical assessments and reassessments for referred customers. The role requires reviewing medical and health-related documents, conducting bio psychosocial assessments, and performing mental health evaluations. The counselor will identify customer functional limitations and make determinations regarding their functional capacity. Coordination with other professionals and ensuring customer outreach for follow-up activities are also key responsibilities. The position emphasizes collaboration with service track staff and participation in team meetings to enhance clinical assessment processes.

Requirements

  • Must be a licensed or certified health professional: LCSW, CRC, LMHC, or other approved credentials.
  • Four years clinical assessment experience including document review and bio psychosocial assessments.
  • Experience in a fast-paced, team environment that is performance driven.
  • Exceptional customer service and engagement skills.
  • Training or experience in motivational interviewing and care management techniques.
  • Excellent organizational skills and strong attention to detail.
  • Experience with publicly-funded programs; TANF experience preferable.
  • Knowledge of laws and regulatory requirements related to program responsibilities.
  • Ability to use various electronic systems to carry out duties.

Responsibilities

  • Conduct clinical assessments and reassessments with referred customers.
  • Review medical and other health-related documents and conditions using standard procedures.
  • Conduct bio psychosocial assessments using approved questionnaires and techniques.
  • Conduct the Social Security Administration sequential evaluation process for determining disabilities.
  • Conduct mental health evaluations for customers identified as in need of evaluation.
  • Identify customer functional limitations and make a functional capacity outcome determination.
  • Request or conduct additional medical or other assessments to support functional capacity outcome determinations.
  • Coordinate Review Physician activities as required.
  • Coordinate Evaluating Clinician activities as needed.
  • Work with the Engagement Specialist to ensure customer outreach for follow-up activities.
  • Compile clinical assessment forms and documentation and prepare clinical assessment reports.
  • Sign off on and gain customer signatures on clinical assessments.
  • Collaborate with service track staff to interpret clinical assessment findings.
  • Use motivational interviewing techniques to elicit complete information from customers.
  • Participate in team meetings to evaluate and improve clinical assessment processes.
  • Participate in required training related to the position.
