Requirements

  • Master’s degree in Social Work accredited by the Council on Social Work Education.
  • Active license as a clinical Social Worker by the Board of Behavioral Sciences. If you reside in Oregon or Washington an LCSW or AWS is required.
  • Minimum of 1 year of experience in healthcare, with preference given to candidates with home health care experience.
  • Current BLS, hands-on CPR certification.
  • Valid driver's license and reliable, insured transportation.
  • Excellent verbal and written communication skills, along with strong organizational abilities.

Responsibilities

  • Conduct psychosocial assessments of patients to understand their needs within the context of their illness and environment.
  • Develop and implement tailored intervention plans based on assessment findings.
  • Maintain accurate clinical records for all patients referred to social work services.
  • Offer information and referral services to patients, families, and caregivers regarding practical and environmental needs.
  • Serve as a liaison between patients, families, caregivers, and community agencies to facilitate access to support services.
