About The Position

The Licensed Masters Social Worker (LMSW) provides licensed, master’s-level social work oversight for the HomeCare and Hospice programs in compliance with the CMS Conditions of Participation and Michigan social work licensure and title requirements. This position fulfills the federal requirement for medical social work services under CMS regulations through a Michigan-licensed, Master’s-prepared social worker, and serves as the Social Worker of Record for hospice patients. The LMSW provides clinical oversight, supervision, consultation, and program leadership to Social Work Assistants (SWA), participates in interdisciplinary care planning, and ensures regulatory compliance for psychosocial and bereavement services. The role focuses on oversight, collaboration, documentation review, quality improvement, and program development rather than routine in-home patient visits. This is a fully remote position. While day-to-day work is performed remotely/virtually, the employee may be required to attend onsite meetings or trainings on an occasional basis.

Requirements

  • Master’s degree in social work (MSW) from a Council on Social Work Education (CSWE) accredited program
  • Current, active Michigan Licensed Masters Social Worker (LMSW) license required
  • Meets CMS Conditions of Participation requirements for provision of medical social work services in Home Health and Hospice
  • Knowledge of Michigan social work licensure law and scope of practice
  • Proficiency with electronic health records and virtual collaboration platforms (e.g., Microsoft Teams)
  • Ability to work independently and effectively in a remote environment
  • Current CPR certification required (may require limited on-site attendance for certification or renewal).

Nice To Haves

  • Minimum of one (1) year of experience in a healthcare, home health, or hospice setting preferred
  • Experience providing clinical oversight, supervision, or consultation preferred

Responsibilities

  • Clinical Oversight & Supervision Provide licensed social work oversight to Social Work Assistants (SWA) providing psychosocial and bereavement services.
  • Conduct scheduled virtual supervision and collaboration meetings with SWA to review cases, documentation, assessments, and plans of care.
  • Review, approve, and co-sign psychosocial assessments, documentation, and discharge summaries as required.
  • Serve as the Social Worker of Record for hospice patients in compliance with CMS and Michigan requirements.
  • Participate remotely in Hospice Interdisciplinary Group (IDG) meetings or provide documented review and input in advance of IDG.
  • Collaborate with nursing, medical providers, spiritual care, and other disciplines to support comprehensive patient-centered care.
  • Provide consultative guidance regarding psychosocial, caregiver, and environmental factors impacting patient outcomes.
  • Provide licensed oversight of the Hospice Bereavement Program, including assessment, risk stratification, care planning, and follow-up processes.
  • Review bereavement assessments and care plans completed by SWA to ensure regulatory and program compliance.
  • Provide consultation regarding bereavement services, memorial activities, and ongoing family support.
  • Review and approve psychosocial components of the interdisciplinary plan of care, including the appropriate use of volunteer services when related to patient or family psychosocial needs.
  • Assist in the development and maintenance of oversight tools, supervision logs, documentation standards, and workflows.
  • Participate in Quality Assessment and Performance Improvement (QAPI) activities related to psychosocial and bereavement services.
  • Provide feedback and recommendations to leadership regarding documentation trends, regulatory compliance, and opportunities for improvement.
  • Maintain current knowledge of federal and Michigan regulations affecting medical social work practice.
  • Maintain active Michigan LMSW licensure and adhere to the NASW Code of Ethics.
  • Participate in required meetings, in-services, and continuing education activities
  • Maintain patient and organizational confidentiality in accordance with HIPAA and organizational policy.
  • Remain flexible to changes in program needs, policy, and regulatory requirements.
  • Perform other related duties as assigned to support HomeCare & Hospice operations.
