Provide on-site rehabilitative services for those experiencing severe and persistent mental illness and/or substance use disorders. Assist clients in obtaining and implementing skills for coping with issues such as trauma, familial and/or peer conflict, mental health disorders, substance abuse, grief and loss, relational issues and community re-engagement. Provide individual, couple, family and group counseling as assigned. Assess patients continuing care needs via chart review, direct interview, and completion of the Treatment Needs Assessment Collaborate with other clinic staff in the development and implementation of appropriate treatment plans and interventions. Generate appropriate medical chart documentation. Assist patients in the completion of the discharge planning worksheet and provide collaborative linkages with other community programs and services. Schedule and coordinate aftercare appointments/activities Communicate via a written report - daily discharge planning activities. Participate in case staffing and consultation with appropriate clinic staff. Work as part of a team and exhibit competent and effective communication skills. Demonstrate awareness and sensitivity to cultural issues and community practices of children and families. Ability to multi-task, manage time, meet deadlines and remain calm and professional under pressure. Maintain standards of confidentiality of CC Idaho clients and records Maintain strict confidentiality and compliance with HIPAA standards and other grant requirements. Perform all work duties and activities in compliance with CC Idaho, Joint Commission, OSHA and HIPAA regulations.
