The Licensed Master Social Worker provides assistance to patients and families in identifying social, emotional, environmental, and financial needs. This role addresses these needs directly or through referrals, connecting patients with available resources and services within the clinic or community. The Licensed Master Social Worker conducts psychosocial assessments, provides counseling and interventions, and collaborates with the care team to address the psychosocial needs of patients and their families.

Requirements

  • Master’s degree from an accredited School of Social Work.
  • One (1) year of professional practice post-graduation.
  • Licensed Master Social Worker (LMSW).
  • Knowledge of oncology disease processes and their effects on patients and families.
  • Knowledge of insurance, state and federal regulations, and resources specific to oncology.
  • Knowledge of social work theory, crisis intervention, and individual, group, and family therapy.
  • Excellent organizational and follow-up skills.
  • Strong verbal and written communication skills.
  • Strong problem-solving abilities.
  • Ability to organize and prioritize work assignments in a fast-paced environment.
  • Ability to analyze situations and respond in a timely manner.
  • Ability to work effectively within multi-functional teams.
  • Ability to establish and maintain effective working relationships within West Cancer Center.
  • Proficiency in computer applications including word processing and e-mail.

Nice To Haves

  • Experience in Oncology or a healthcare setting.

Responsibilities

  • Gather and assess information regarding patients’ physical needs, mental status, family support, financial resources, and potential barriers to care.
  • Conduct psychosocial assessments of patients and families and develop plans to address identified needs.
  • Develop personalized care plans in coordination with the patient, care providers, and patient support team.
  • Provide direct interventions including individual, family, or group counseling and crisis intervention as needed.
  • Provide psycho-social support to patients and their families or caregivers.
  • Act as an advocate for patients and families.
  • Remain current on community and governmental resources and refer patients to appropriate programs as needed.
  • Maintain appropriate documentation and statistical data.
  • Communicate with physicians and nurses regarding psychosocial health issues and patient/family needs.
  • Interface with community organizations and programs to support patient resources.
  • Maintain patient dignity, confidentiality, professionalism, and compliance with clinic policies.
  • Attend staff meetings as scheduled and participate in a team-oriented environment.
  • Perform other duties as assigned.
