Requirements
- MSW from CSWE-accredited program
- Active LMSW or LCSW (or eligibility)
- 2+ years healthcare or medical social work experience
- Strong case management and communication skills
- Knowledge of HIPAA and ethical standards
Responsibilities
- Conduct psychosocial assessments
- Provide counseling and crisis intervention
- Coordinate care with interdisciplinary teams
- Assist with discharge planning
- Connect patients to community resources
- Maintain accurate, compliant documentation
