This position is responsible for providing direct patient/family care, as well as engaging in program development and interdisciplinary team development and additional departmental duties. The BHC functions as an integrated member of the PCMH and assists in managing the overall health of the enrolled population through improved recognition, treatment and management of psychosocial/behavioral problems and psychosocial aspects of medical conditions. BHC providers will demonstrate competency in providing brief evaluations, cognitive-behavioral therapy, motivational interviewing, as well as other evidence-based/evidence informed interventions, to individuals, families and groups, to address behavioral health issues, chronic and acute physical health conditions and substance abuse conditions.
