The Licensed Master Social Worker (functional title: Residential School Transition Coordinator (RSTC)) plays an integral role in facilitating planning for youth aging out of residential schools. The RSTC is responsible for coordinating the transition to adult OPWDD services for students aging out of residential school settings. The RSTC will be part of a team that is expected to assist with this planning for students from across the 18 counties within the Hudson Valley, Taconic, and Capital Districts of Region 3. Across the 18 counties within Region 3, over 200 students will be aging out from residential schools in 2025-2027. This number is expected to grow as additional students are found to be eligible for OPWDD services.
