About The Position

The Licensed Social Worker is a member of the care management team and responsible for assisting in the management of the continuum of care from admission through discharge for assigned patients. The role reflects appropriate knowledge of Social Work scope of practice, current state requirements, CMS Conditions of Participation, EMTALA, Patient Bill of Rights, AB 1203 and other Federal or State regulatory agency requirements specific to the Social Work role. The Social Work team member will partner with the multi-disciplinary team utilizing best practices, relevant data and compliance with the mission/philosophy, standards, goals and core values of UAMS: Comfort, Hope, and Healing. Primary elements of the social work role include: assessment of the psychological, financial, and discharge planning needs of the patients and families, providing information, support, counseling, and referrals to other levels of care and appropriate resources for a variety of age-specific populations, including neonatal, pediatric, adolescents, adults, and geriatric.

Requirements

  • Masters' degree in Social Work
  • Must be licensed by an Arkansas Professional Board as a Licensed Master Social Worker (LMSW) or Licensed Certified Social Worker (LCSW) by date of hire (Provisional License Master Social Worker accepted)
  • Work the equivalent of 24 hours on a regularly scheduled basis (monthly).
  • Work 1-2 designated UAMS holidays (excluding Birthdays) and four weekend shifts a year (1 per quarter) based upon the needs of the department.
  • Attend HR Orientation and complete all HR new hire requirements.
  • Department orientation must be scheduled and completed within 90 days of hire.
  • PRN staff must comply with all departmental scheduling procedures, attendance policies, and clinical performance standards.
  • PRN staff are only allowed to work 1000 hours in a 12 month rolling period, which begins at orientation and includes any educational hours.
  • PRN staff assignments/shifts are subject to cancellation and the PRN employee will be notified by the Manager or designee at least one hour prior to the beginning of the shift.

Nice To Haves

  • Certified Case Manager (CCM) or American Case Management (ACM) certification.
  • 2 years experience in an Acute Care Hospital Setting
  • Experience in Case Management

Responsibilities

  • Completes assessment for patients admitted to the hospital.
  • Performs discharge planning and aftercare coordination, community referrals and linkages.
  • Provides patient and family education and adjustment to illness counseling, grief counseling and crisis intervention.
  • Promotes patient and family centered care and documents the patient choices in the medical record as required.
  • Recommends care and services that are appropriate to the specified chronological age, developmental age, social maturity, and cultural background of the patient being treated.
  • Documents and communicates with all team, physicians, and patient family concerns pertaining to the coordination of care and services.
  • Serves as a consultant for processing guardianship, power of attorney, health care proxy, and advance directives.
  • Collaborates with other UAMS Departments and actively participates in hospital wide performance improvement metrics and regulatory compliance initiatives.
  • Reports and reviews department specific data metrics on a monthly basis.
  • Implements, monitors, and evaluates the Care Management quality improvement initiatives.
  • Interacts with and assists the public in a professional and friendly manner as needed.
  • Demonstrates effective communication skills; communicates accurate and complete information; maintains strict confidentiality when necessary.
  • Demonstrates positive working relationships with co-workers, management team, and ancillary departments; follows the Core Concepts of Patient and Family Centered Care and the Basic Code of Conduct Guidelines.
  • Assist internal and external stakeholders as needed with exceptional customer service.
  • Other duties as assigned in order to meet the UAMS Core Values and the business needs of the Department
