About The Position
As a Licensed Master Social Worker, you will be part of a multidisciplinary team that provides culturally competent and evidence-based treatment and support services in a recovery-oriented environment to individuals with mental illness. Duties include providing crisis intervention, service linkages, and discharge planning; developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed. You will also determine discharge needs and appropriate community resources, and establish and maintain relationships with community providers to ensure continuity of care.
