About The Position

"Navitas Healthcare, LLC" is seeking an experienced Licensed Master Social Worker (LMSW) to support inpatient medical services within a hospital setting. The ideal candidate will have prior hospital-based experience and be comfortable working in a fast-paced, interdisciplinary healthcare environment, particularly across medicine, surgery (med/surg), and rehabilitation units

Requirements

• Minimum of 1 year of experience as a Licensed Master Social Worker (LMSW)
• Hospital-based experience required
• Experience working in inpatient med/surg and rehabilitation settings
• Strong assessment, communication, and documentation skills
• Ability to function effectively within a multidisciplinary healthcare team
• Master’s Degree in Social Work (required)
• Active Licensed Master Social Worker (LMSW) license issued by New York State Education Department (NYSED)
• Primary Source Verification required

Responsibilities

• Provide comprehensive social work services to patients in inpatient medicine, surgery, and rehabilitation units
• Conduct psychosocial assessments and develop individualized care plans
• Collaborate with physicians, nurses, case managers, and interdisciplinary care teams
• Support patients and families with discharge planning and coordination of community resources
• Maintain accurate and timely documentation in compliance with hospital policies and regulatory standards
• Ensure patient confidentiality and adherence to all organizational policies and procedures
• Perform additional duties as assigned
