About The Position

We are seeking a Licensed Master Social Worker (LMSW) to provide comprehensive social work services in an acute care setting. The LMSW will support patients and families by addressing psychosocial needs, coordinating care, and facilitating discharge planning to ensure optimal patient outcomes.

Requirements

• Active Licensed Master Social Worker (LMSW) license issued by NYSED (Primary Source Verification required)
• Master’s Degree in Social Work (MSW) required
• Minimum 1 year of LMSW experience required
• Acute care or inpatient (medicine/surgery/med-surg) experience required
• Strong communication, organizational, and interpersonal skills
• Ability to work in a fast-paced healthcare environment

Responsibilities

• Conduct psychosocial assessments for patients in inpatient settings
• Provide counseling and emotional support to patients and families
• Develop and implement discharge and care plans
• Collaborate with physicians, nurses, and interdisciplinary teams
• Assist with care coordination, referrals, and resource planning
• Support patients with medical, social, and financial challenges
• Maintain accurate and timely documentation
• Ensure compliance with hospital policies and regulatory standards
• Participate in care team meetings and case discussions
