About The Position

We are seeking a skilled and compassionate Licensed Master Social Worker (LMSW) for a 13-week hospital-based contract assignment in New York, NY. The LMSW will provide clinical social work services across inpatient medicine, surgery (med/surg), and rehabilitation units. This role focuses on psychosocial assessment, discharge planning, care coordination, and supporting patients and families throughout hospitalization and transition of care.

Requirements

• Master’s Degree in Social Work
• Valid Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (Primary Source Verification required)
• Minimum 1 year of LMSW experience
• Strong clinical competency in hospital or healthcare settings
• Ability to work effectively within an interdisciplinary team

Responsibilities

• Conduct comprehensive psychosocial assessments for patients and families
• Develop and implement discharge plans in collaboration with interdisciplinary teams
• Coordinate post-hospital services including rehabilitation, home care, and community support
• Provide counseling, crisis intervention, and emotional support
• Assist patients with accessing community resources, housing assistance, and financial support programs
• Maintain accurate documentation in accordance with hospital policies and regulatory standards
• Facilitate safe transitions to rehabilitation facilities or other levels of care
• Collaborate closely with physicians, nurses, case managers, and allied health professionals
