About The Position
An acute care hospital in New York City is seeking a Licensed Master Social Worker (LMSW) to provide comprehensive social work services to adult inpatient populations . This role focuses on psychosocial assessments, care coordination, crisis intervention, and discharge planning within a hospital-based, interdisciplinary care environment. The LMSW will work onsite and collaborate closely with clinical teams to ensure safe, effective, and patient-centered care.
Requirements
- Minimum 1 year of experience as a Licensed Master Social Worker (LMSW)
- Master’s Degree in Social Work
- Active New York State Licensed Master Social Worker (LMSW) license
- Primary Source Verification required
- Strong clinical assessment, communication, and documentation skills
- Onsite position – no remote option available (0% remote)
Nice To Haves
- Hospital experience preferred
- Adult inpatient psychiatry experience preferred
Responsibilities
- Conduct psychosocial assessments for adult inpatient populations
- Develop and implement individualized care and discharge plans
- Provide patient and family counseling and crisis intervention
- Coordinate discharge planning and referrals to appropriate community resources
- Collaborate with physicians, nurses, and allied health professionals
- Maintain accurate, timely, and compliant documentation per hospital policies
- Participate in interdisciplinary rounds and case conferences
