About The Position

"Navitas Healthcare, LLC" is seeking a qualified Licensed Master Social Worker (LMSW) to support inpatient care and discharge planning in a hospital setting. This role requires strong clinical judgment, excellent communication skills, and the ability to manage a dynamic caseload effectively.

Requirements

• Master’s Degree in Social Work (MSW) – Required
• Active Licensed Master Social Worker (LMSW) – State of New York
• Strong knowledge of psychosocial assessment and case management practices
• Excellent communication and interpersonal skills
• COVID-19 Vaccination – Required (medical exemptions only)
• Influenza Vaccination – Required (medical/religious exemptions and declinations allowed)

Nice To Haves

• Experience with EMR systems such as EPIC or Allscripts
• Background in hospice, hemodialysis, or substance abuse cases
• Experience managing guardianship and ethical case scenarios

Responsibilities

• Conduct comprehensive psychosocial assessments for patients
• Manage inpatient discharge planning and care coordination
• Facilitate referrals for post-discharge services and community resources
• Maintain and manage a caseload of approximately 25–30 patients
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care
• Document patient interactions, assessments, and care plans accurately
• Provide support to patients and families during care transitions
• Handle complex cases, including guardianship and ethical concerns
