About The Position

We are seeking a Licensed Master Social Worker (LMSW) to support inpatient medical services in a hospital setting. The LMSW will collaborate with interdisciplinary care teams to provide psychosocial assessments, discharge planning, and patient advocacy while ensuring continuity of care and compliance with regulatory standards.

Requirements

• Master’s Degree in Social Work (MSW)
• Active New York Licensed Master Social Worker (LMSW) license issued by NYSED
• Minimum 1 year of LMSW experience
• Hospital or inpatient experience required

Responsibilities

• Conduct comprehensive psychosocial assessments for hospitalized patients
• Provide discharge planning and coordination of post-acute services
• Collaborate with physicians, nurses, case managers, and interdisciplinary teams
• Support patients and families with education, resources, and care planning
• Address barriers to discharge and facilitate safe transitions of care
• Document all assessments and interventions accurately in the electronic medical record
• Ensure compliance with hospital policies, ethical standards, and regulatory requirements
