Requirements

• Master’s Degree in Social Work (MSW).
• Active New York State Licensed Master Social Worker (LMSW) license (Primary Source Verification required).
• Minimum one (1) year of LMSW experience.
• Experience working with adults with mental illness.
• Strong crisis intervention and communication skills.

• Hospital-based social work experience.
• Experience in large urban healthcare systems.
• Familiarity with discharge planning in acute care settings.

Responsibilities

• Conduct comprehensive psychosocial assessments for adult patients.
• Develop, implement, and monitor individualized treatment and discharge plans.
• Provide crisis intervention and supportive counseling services.
• Collaborate with interdisciplinary teams including physicians, nurses, and case managers.
• Coordinate referrals to community-based services and outpatient programs.
• Maintain accurate and timely clinical documentation.
• Advocate for patients to ensure continuity and quality of care.
• Participate in case conferences and team meetings.
• Ensure compliance with hospital policies and New York State licensing standards.
