About The Position

As the Master Social worker for Banner Home Health, you will see Home Health Patients. You will assist patients and families to identify resources in the community. Help patients access these resources. Assist patients and family with disease progression and understanding the disease process. Help patient and family identify supports system. Assist families with understanding care giver burnout. Help the patient with motivational interviewing to make informed decisions about their health care. You will work alongside a team of nurses, and therapist to help the patient regain prior level of functioning. This position will provide home visits to patients in Central Phoenix including the following primary areas: New River, Anthem, Cave Creek, Scottsdale, Phoenix, Glendale, and Laveen.

Requirements

  • Requires a Master's Degree in Social Work, Counseling or related field.
  • Requires a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
  • Requires a proficiency level typically achieved with 3 years acute care hospital experience.
  • Must have knowledge of government/community resources such as Medicare, Medicaid, long-term care or any other applicable resources/services.
  • Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, human relations skills and time management skills.

  • Additional related education and/or experience preferred.

Responsibilities

  • Processes and facilitates the assessment with analysis of functional and psychological needs of the patient.
  • Formulates a plan of intervention acceptable to the patient, family, and health care team.
  • Documents all interventions in the patient medical record both timely and accurately.
  • Provides advocacy, assistance, support, counseling and crisis intervention to patients and families.
  • Functions in a liaison role between the hospital and community in making community resources available to the patient and family.
  • Maintains knowledge of Medicare, Medicaid and other program benefits to assist patients with discharge planning and choices.
  • Gather / assessing information, apply counseling and developmental theories, utilizing diagnostic frameworks, and engage in collaborative treatment planning.
