About The Position

The role of the Primary Care Licensed Independent Social Worker is part of the primary care within the primary care setting. Duties and responsibilities are commensurate with education and licensure. The Licensed Independent Social Worker provides psychoeducation strategies and develops specific behavioral change plans, develops and discusses proposed treatment plans (including adjustments as necessary) for patients, engages in behavioral health protocols for target populations through interviewing patients/families/care team, and monitors and documents progress. Requirements

  • Master's or Doctorate degree in social work from an accredited program
  • Minimum of 2-5 years of relevant behavioral social work experience preferred
  • Computer skills to include proficiency in MS Word, Excel and PowerPoint.
  • Working knowledge of an electronic medical record application.
  • A team player that can follow processes to achieve a common goal.
  • Highly organized and well-developed oral and written communication skills.
  • Must be able to analyze and interpret data.
  • Licensed Independent Social Worker-Clinical Practice (LISW-CP) required
  • Current applicable South Carolina license
  • Continuous requirements are to perform job functions while standing, walking and sitting.
  • Ability to bend at the waist, kneel, climb stairs, reach in all directions, fully use both hands and legs, possess good finger dexterity, perform repetitive motions with hands/wrists/elbows and shoulders, reach in all directions.
  • Maintain 20/40 vision corrected, see and recognize objects close at hand and at a distance, work in a latex-safe environment and work indoors.
  • Infrequently lift and/or carry objects weighing 20 lbs (+/-) unassisted.
  • Infrequently lift from 36” to overhead 15 lbs.
  • Infrequently work in dusty areas and confined/cramped spaces.

Responsibilities

  • Patient contact and appointments to provide psychoeducation, counseling, and crisis intervention.
  • Ability to work with patients via direct contact and telephonically.
  • Ability to make prompt and accurate clinical and psychosocial assessments of behavioral conditions and determine patient needs.
  • Ability to design and implement pathways and protocols for behavioral conditions.
  • Short-term counseling for patients related to identified needs (in-person, by telephone, and by secure video link; will deliver both individual level counseling as appropriate).
  • Resource to providers, clinical staff, and care coordinators.
  • Ability to work effectively as a team interacting with patients, primary care providers, site staff, and PCMH team.
  • Working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
  • Act as a clinical resource and provide behavioral health education updates.
  • High level of comfort working with primary care providers using a consultative style.
  • Participate in patient care conferences with providers, site staff, and PCMH team as appropriate.
  • Facilitate appropriate patient referrals to other healthcare providers and community resources.
  • Develop and maintain relationship with healthcare and community resource providers.
  • Other duties as assigned
