About The Position

The position involves providing psychosocial assessments of individuals and families, focusing on their limitations, deficits, and strengths in their current situations and environments. The LICSW will develop treatment plans with appropriate interventions and provide services tailored to meet patients' specific mental health and substance abuse needs. Responsibilities include engaging patients in care planning, maintaining community resource inventories, assessing patients' knowledge of their medical conditions, and providing direct care management and clinical consultation. The role also includes delivering therapeutic interventions, documenting service delivery, and promoting positive inter-department cooperation.

Requirements

  • Master's degree from a CSWE accredited program or a Ph.D. or DSW degree in social work.
  • Licensed by the State Board of Social Work.
  • Two years of supervised social work experience in a health care setting.
  • Experience working with individuals who have chemical dependencies is preferred.
  • Experience with Motivational Interviewing, Self-Management, and Patient Engagement strategies.
  • Demonstrated experience in working with an underserved population.

Nice To Haves

  • Certified Clinical Alcohol, Tobacco, and Other Drug Social Worker (C-CATODSW) is highly preferred.
  • Experience with Medicare and Medicaid populations and insurance providers.

Responsibilities

  • Engage patients and caregivers telephonically and in-person in active care planning.
  • Create and maintain a comprehensive inventory of local community resources.
  • Assess the patient's knowledge of their medical condition and provide education and self-management support.
  • Provide direct care management to a high-risk population and clinical consultation to the care team.
  • Deliver psychosocial diagnostic assessments and treatment recommendations.
  • Provide in-office mental health and/or substance abuse intervention services.
  • Continually assess patient needs and appropriateness of treatment plans.
  • Document service delivery, including treatment goals and progress notes.
  • Promote positive inter-department cooperation and participate in committees.
  • Assist patients and family members with concerns and maintain confidentiality.
