Our Behavioral Health team provides compassionate, evidence-based care to individuals and families of all ages. We focus on integrated, patient-centered care in collaboration with our primary care and specialty providers, ensuring continuity of mental and physical health services. Provide integrated, clinic-based behavioral health assessments and therapy services to children, adults, and families with a focus on improving emotional well-being and overall health outcomes. This is a full-time position working day shift - 8:00am-5:00pm.

Requirements

  • Master’s degree in Social Work or Marriage and Family Therapy
  • Licensed Independent Clinical Social Worker (LICSW) or Licensed Marriage and Family Therapist (LMFT) in the state of Minnesota

Responsibilities

  • Conduct comprehensive intake assessments to evaluate patient needs, functioning, and treatment goals
  • Provide individual, group, and family therapy for patients referred by LHS clinic providers
  • Develop and update treatment plans based on ongoing assessments
  • Collaborate with the Behavioral Health Services Admissions Specialist for scheduling, utilization review, and insurance authorization
  • Maintain timely and accurate documentation in the Electronic Medical Record (EMR) system (within five days of service)
  • Respond effectively to patient crises and develop individualized intervention strategies
  • Uphold patient confidentiality and adhere to records release protocols and licensing requirements
  • Participate in clinical consultations, staff meetings, and team collaboration
  • Deliver educational in-services to staff, the professional community, or public as directed
  • Demonstrate empathy, professionalism, and strong communication in patient and team interactions
