Job Summary The Licensed CNA (Certified Nursing Assistant) provides direct patient care to hospice and home health patients in their home or place of residence. The CNA assists in providing a clean and safe environment. They work cooperatively with patients, families, and caregivers and share observations and changes with the RN care manager. They work under the supervision of a Registered Nurse. This position plays a critical role in supporting Augusta Health’s mission and advancing departmental goals through measurable performance indicators and service excellence. This position contributes to a collaborative, patient-centered environment and helps drive outcomes aligned with organizational priorities.

Requirements

  • Education: High School Diploma or equivalent
  • Virginia State Approved CNA program certification
  • Educational requirements per hospital certifications (Example: Stroke, Orthopedic/Spine)
  • Licensure/Certification: Current Virginia State CNA License
  • Basic Life Support - BLS
  • Experience: None
  • Driver's License: Yes
  • To drive an Augusta Health Vehicle, must also be 21 years old and Motor Vehicle Record (MVR) must be approved by Risk Management.
  • Eligibility to work in the United States and meet Virginia state employment requirements
  • Must be ablet to read, write, and verbally report clinical information to patients, representatives, and caregivers, as well as other hospice team members
  • Demonstrates compassion for patients at end of life
  • Ability to work in patients’ homes in various conditions
  • Ability to work a flexible schedule
  • Ability to travel to and from patient homes in various weather conditions
  • 12 hours of additional in-service per year

Responsibilities

  • Follows established plan of care for patient.
  • Reports changes in patient condition promptly to the registered nurse.
  • Provides direct patient care under the direction of the RN, meeting patient’s hygiene and comfort needs, per plan of care.
  • Cares for patients of various ages.
  • Assists patients with personal hygiene, including shower, tub or bed baths and mouth, skin and hair care as defined in the plan of care.
  • Takes and records temperature, blood pressure, pulse and respirations as assigned
  • Provides housekeeping services including meal preparation and feeding, when required per plan of care.
  • Provides skills necessary to assist the patient with proper nutrition and fluid intake.
  • Monitors patient’s condition and environment, reporting changes to the registered nurse.
  • Records and reports changes observed to the registered nurse.
  • Information is obtained and relayed in a safe and appropriate fashion.
  • Collects and delivers patient specimens, as assigned.
  • Secures and delivers patient supplies and groceries, as assigned.
  • Assists patients with self-administered of oral medications, as assigned.
  • Provides a clean, safe, and comfortable environment.
  • Provides an environment which promotes respect for the patient, privacy, and property.
  • Assists patients in and out of bed, including the use of mechanical lifting equipment, within the plan of care.
  • Promotes positive, supportive, and respectful communication with patients, families, caregivers, and team members.
  • Demonstrates effective time management.
  • Promotes hospice philosophy to ensure quality of care.
  • Seeks counsel of a clinical professional when needed.
  • Recognizes extent of own knowledge and refers problems appropriately.
  • Assists with exercise (assists with prescribed exercises when the patient and the nursing assistant have been instructed by the appropriate health professional.) walking, including the use of walkers, and wheelchairs, when applicable), elimination, t(use of toilet facilities, including bed pans) therapeutic measures, and nutrition, as assigned.
  • Completes documentation for electronic chart, at the point of care, as well as required data for billing of patient visit
  • Information is accurate, appropriate and received in a timely fashion, per agency policy.
  • Calls office when patient not at home.
  • Calls office when finished earlier than scheduled.
  • Uses time and travel efficiently.
