We offer a variety of programs including inpatient and outpatient behavioral health, residential treatment for court-involved teenage girls, geriatric services, clinical day treatment schools and chemical dependency services. Program Summary Natchaug Hospital's Journey House program is a specialized residential treatment facility on the Mansfield campus for adolescent girls with identified needs for behavioral health services who admitted through a referral from the juvenile justice system. Journey House offers a variety of evidence-based treatment modalities that are carried out in individual, group and family settings. Residents and the treatment team collaborate on discharge planning, which includes coordination of individual and family therapy, medication management, identification of a primary care physician and any vocational, recreational or educational planning. Job Summary Licensed Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in Journey House. Clinical services include individual, group and family therapy. Key responsibilities of Licensed Behavioral Health Clinician: Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate. Provides direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end -of-life care, referral and discharge planning. Refers patient to appropriate levels of care and available resources that promote the individualized care plan. Mains timely patient-centered documentation that reflect the patient and client systems’ pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies. Independently able to provide clinical social work interventions to all patient populations. Provide social work field supervision to second year social work students. Behavioral Health Clinicians are eligible for a Sign On Bonus up to $2,500.00, in accordance with program guidelines

Requirements

  • Master’s degree in social work, or counseling, or marriage and family therapy or related Human Service field.
  • Previous experience in a healthcare setting required, preferably working with a chemical dependency population, child/adolescents preferred.
  • Current Connecticut LCSW, LPC, LMFT, or Psychologist.
  • Excellent communication skills, with the ability to establish relationships within and outside their entity
  • Is cognizant of health-related social needs and cultural competence. Recognizes and addresses health disparities-inequalities and injustices directed toward clients, related to access to care and provision of health services.
  • Valid Driver's License for a minimum of 3 consecutive years.
  • Ability to obtain a Public Service License Endorsement 'A', which requires passing a drug screen and a DOT physical and having a valid drivers license for at least 3 consecuitve years.
