Licensed Clinical Social Worker (LCSW) – PRN Rehab Without Walls® | Home & Community Neurorehabilitation 📍 Oakland, CA Elevate Your Career & Make an Impact Where Life Happens Rehab Without Walls® is seeking an experienced Licensed Clinical Social Worker (LCSW) to join our Home & Community–based Neurorehabilitation Program in San Francisco. This PRN opportunity offers meaningful flexibility while allowing you to deliver impactful, relationship-driven care in the settings that matter most, such as patients’ homes and communities . Who We’re Looking For: An experienced LCSW passionate about providing counseling services that support an individual’s physical, mental, cognitive, and emotional adjustment throughout recovery. A clinician skilled in delivering therapeutic interventions and resource recommendations that help patients achieve personal, career, and independent living goals in the most integrated settings possible. A professional who values autonomy , communication , and collaboration to maximize patient outcomes. A team-oriented clinician who enjoys working alongside a diverse group of providers, including Occupational Therapists, Physical Therapists, Speech-Language Pathologists, Music Therapists , and other rehabilitation professionals in the San Francisco area. Requirements
- Master's Degree in Clinical Social Work (LCSW)
- Current Licensed Clinical Social Worker Licensure in the state of practice
- Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or acute diagnosis
- CPR certification required
- Ability to lift 50 pounds
- Moving,lifting,or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
- Duties require fine motor skills ,visual acuity, and walking/ standing for extended periods
- Additional physical requirements include:pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
Responsibilities
- Performs comprehensive evaluations, assessing the social, emotional and support needs of the patients and their families
- Sets measurable objectives that are formulated in conjunction with the rehab team
- Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient's health problems
- Instructs/counsels’ patients and families in treating and coping with social and emotional response connected with illnesses
- Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs
- Identifies and assists the patient/ and or family to utilize appropriate community resources to achieve measurable objectives
