Requirements

  • LCSW or LMFT licensure required
  • Master’s Degree in Social Work, Counseling, Marriage and Family Therapy and/or Psychology required
  • Availability to be on call 24-hours for crisis management
  • At least two years of experience supervising clinical/case management staff
  • Pass a Security/background investigation via National Agency Check with Written Inquiries (NACI) and a Special Agreement Check (SAC) which includes electronic fingerprinting
  • Must qualify to complete the credentialing process through the VA’s VetPro system
  • Excellent oral and written communication skills
  • Proficiency in computer and information systems
  • Professional manner and appearance
  • Experience working with homeless or similar population
  • Ability to take general direction, work independently and within a team
  • Ability to work effectively with diverse group of clients, staff, and community members
  • Must have a valid driver’s license, meet company insurance requirements, complete a provided driver training course and drive company vehicles for work-related activities to include client transportation

Responsibilities

  • Oversees and manages HUD-VASH Case Managers and HUD-VASH Peer Specialists
  • Serves as the program’s Mental Health Treatment Coordinator
  • Oversees client HUD-VASH screening and intake for approval
  • Directly supervises HUD-VASH Case Managers for residential programs and other specialized program staff depending on site specifications
  • Reviews and approves clients’ Individual Housing and Employment Plans (IHEP) and Mental Health treatment plans (MHTP)
  • Reviews and approves the Level of Care (LOC) case management stages
  • Provides non-clinical supervision to non-clinical, non-licensed Contractor staff providing related supportive services
  • Collaborates with on-site treatment providers, and VA SSVF, GPD, and HUD-VASH Liaisons
  • Ensures adherence to clinically-based programmatic protocols and quality standards are met
  • Communicates in a professional, tactful, flexible, and mindful manner while being aware of the local environment on the ground when interfacing with clients and partners alike
  • Utilizes clinical judgement
  • Collaborates with the VA COR/Liaison and multidisciplinary teams
  • Monitors Case Manager caseloads
  • Ensures that confidential and complete client records are securely maintained
  • Monitors the program, tracking systems, metrics, and outcomes for all assigned clients in HUD-VASH
  • Implements relevant clinically-based Case Management processes
  • Assists in implementing procedures needed to obtain CARF and/or any other accreditation
  • Supervises the treatment curriculum governing client groups or classes
  • Performs periodic case file reviews and provides guidance and corrective action to Case Management staff
  • Collaborates with U.S.VETS sections and personnel to bring and monitor necessary services onsite for clients
  • Facilitates weekly clinical and Case Management meetings
  • Attends assigned agency clinical meetings and stakeholder meetings as needed
  • Conducts quarterly individual supervision with front line staff
  • Creates and implements a yearly training schedule for all site staff. This includes monthly training for all staff and individualized training for each position. This schedule should be submitted by June 15 of every year to be adhered to for the next fiscal year
  • Provides a monthly educational training to all staff on different clinical expertise (this can be included in the yearly training schedule)
  • Serves as a Continuing Education Coordinator for the site
  • Analyzes reporting, and notes programmatic trends that inform strategic planning
  • Coordinates all discharges before they occur, and signs off for completion
  • Performs other duties as required
