About The Position
The clinical Social Worker coordinates team-based care to provide health services to individuals through effective partnerships with patients, their caregivers, families, community resources, and providers to achieve patient-family centered care. As a patient care partner, provides a full range of outpatient clinical services, including short-term psychotherapy, group therapy, couples therapy, family therapy, and evaluation and referral to appropriate resources.
