About The Position

Sun River Health provides the highest quality of comprehensive primary, preventive, and behavioral health services to all who seek it, regardless of insurance status and ability to pay, with a particular focus on the underserved and vulnerable populations. We are a Federally Qualified Health Center (FQHC) serving communities across Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia, and Westchester counties. Sun River Health is currently seeking a Licensed Clinical Social Worker (LCSW) to join our team at the Inwood site in Bronx, NY. The Licensed Clinical Social Worker (LCSW) will work as part of an interdisciplinary team to provide specialized knowledge and skills for psychiatric and mental health evaluation, assessment, and management of patients. They will provide crisis intervention, conduct mental health assessments, and offer individual, family, and group counseling services to patients.

Requirements

  • Master's Degree in Social Work (MSW) or related field.
  • Licensed Clinical Social Worker (LCSW) in New York State.
  • CPR/BLS Certification.

Responsibilities

  • Provide a comprehensive mental health intake and assessment for all new patients referred by primary care providers or other service providers for medical and/or social/emotional issues.
  • Participate as a patient advocate in clinical meetings, patient care conferences, and other discussions pertaining to the patient's care with the medical and mental health program staff.
  • Assess the multidimensional needs of patients and develop and implement appropriate care plans.
  • Refer patients in need of more intensive mental health or case management services to the appropriate external resources.
  • Provide individual, family, and group counseling to patients and their families.
  • Offer crisis intervention and emergency evaluations to patients as required.
  • Ensure timely and appropriate follow-up for referred patients.
  • Ensure that the program's clinical services comply with grants, funding sources, and regulatory standards.
  • Participate in and facilitate case conferences within the agency and with external organizations as necessary.
  • Maintain accurate and up-to-date data and case records, preparing necessary reports as needed.
  • Collaborate effectively with the Psychiatric Nurse Practitioner, Psychiatrist, Medical Doctor, and other staff involved in the management of patient care.
  • Develop treatment plans with patients and/or family members in a timely and efficient manner.
  • Ensure the accurate and timely completion of clinical documentation in patient records.
  • Provide patient education regarding psychiatric and mental health issues.
  • Demonstrate knowledge of the principles of growth and development over the lifespan, including identifying and categorizing each patient's age-specific needs, such as those for adolescents, adults, and geriatric patients.
  • Support integrated care through group therapy sessions, both independently and in collaboration with other providers.
