About The Position

Requirements
- Master's
- LCSW (New York license)
- one to two years’ experience providing clinical services in the mental health field
- Bilingual English/Spanish Speaking
Responsibilities
- Assess nature of clients' situations by interviewing and reviewing personal history
- Develop and execute individual treatment plans
- Determine appropriate milestones to gauge client progress
- Communicate client updates to all relevant parties
- Facilitate referrals to other healthcare professionals and programs
- Maintain accurate client documentation
- crisis assessments/triage with appropriate interventions
- provision of other services related to wellness
- other related activities designed to provide integrated health care services
