Requirements
- Active, unencumbered Florida Licensed Clinical Social Worker (LCSW) license
- Minimum two years of clinical counseling experience
- Valid driver’s license, reliable transportation, and auto insurance
Nice To Haves
- Home health or telehealth experience preferred
Responsibilities
- Provide psychosocial assessments, counseling, and behavioral health support
- Develop and coordinate care plans to address social, emotional, and environmental needs
- Connect patients with community resources and support services
- Assist patients and families in navigating social, financial, and healthcare challenges
- Collaborate with physicians and interdisciplinary care teams to support patient outcomes
- Educate patients and caregivers on coping strategies and available resources
- Maintain accurate and timely documentation in accordance with Medicare guidelines
- Document visits efficiently using AI-assisted EMR tools
