The Clinical Social Worker identifies, assesses, and manages the psychosocial needs of patients and families relating to home health services, terminal illness and end of life care, and develops and implements appropriate care plans as part of an interdisciplinary team. Social work services are based upon the patient’s psychosocial assessment and the patient’s and family’s needs and acceptance of these services. Provides patients and families with counseling, bereavement counseling, education, serves as family and patient advocate and arranges consultations when needed related to hospice, palliative care and end of life. Requirements

  • Master of Social Work (MSW) degree from a school accredited by the Council on Social Work Education.
  • Must hold Washington State licensure as one of the following: o Licensed Independent Clinical Social Worker (LICSW) o Associate Licenses (LSWAA or LSWAIC) permitted if completing supervised hours toward full licensure.
  • Minimum of one (1) years’ experience in a health care setting or equivalent experience.
  • CPR certification, as applicable.
  • Demonstrates excellent written and verbal communication skills and strong interpersonal skills.
  • Demonstrates knowledge of resources available in the community.
  • Driving is an essential function and/or a legitimate business purpose of this position; therefore, a valid driver’s license is required as a condition of employment.
  • Giving full attention to what other people are saying, taking time to understand points being made, asking questions as appropriate
  • Near and far vision acuity
  • Ability to sit, stand, bend and twist for prolonged periods or intermittently throughout shift
  • Ability to lift up to 25 pounds

Responsibilities

  • Provide medical social services to Home Health and Hospice patients and their families following established protocols.
  • Identify, assess, and support patients’ emotional, social, spiritual, and cultural needs.
  • Assist patients and families in accessing home-based and community resources.
  • Act as a liaison with external agencies such as SSA, VA, and DSHS.
  • Assess family financial situations and facilitate referrals for financial assistance when appropriate.
  • Conduct initial and ongoing psychosocial assessments for Home Health, Hospice, and bereavement services.
  • Evaluate patient appropriateness for program admission and continued care.
  • Develop social histories and individualized care plans to enhance services.
  • Participate in interdisciplinary team (IDT/IDG) meetings to develop and implement care plans.
  • Facilitate patient care conferences and encourage family participation in care decisions.
  • Plan and coordinate patient discharge to ensure continuity of care.
  • Complete all required documentation accurately and within mandated timeframes.
  • Maintain HIPAA compliance and confidentiality in all records and communications.
  • Generate physician plan-of-care recertifications and required operational reports.
  • Evaluate, document, and report patient care needs and condition changes during each visit.
  • Communicate regularly with patients and supervisors regarding care plans and observations.
  • Educate patients, families, and community partners on hospice, palliative, and end-of-life care.
  • Ensure compliance with quality standards, Medicare requirements, and state and federal regulations.
  • Collaborate with caregivers and service providers to coordinate comprehensive patient care.
  • Maintain professional development through training, continuing education, and active team participation.
