The Clinical Social Worker identifies, assesses, and manages the psychosocial needs of patients and families relating to home health services, terminal illness and end of life care, and develops and implements appropriate care plans as part of an interdisciplinary team. Social work services are based upon the patient’s psychosocial assessment and the patient’s and family’s needs and acceptance of these services. Provides patients and families with counseling, bereavement counseling, education, serves as family and patient advocate and arranges consultations when needed related to hospice, palliative care and end of life.
