Job Summary: Accountable for meeting the psychosocial needs of patients and families through clinical assessment, crisis intervention, referral, care coordination, discharge planning, brief therapy, case consultation with the healthcare team and collaboration with other professionals in the community. This position is a licensed professional and serves as a leader within their profession.
