Job Summary: The Licensed Clinical Social Worker (LCSW) provides trauma-informed psychosocial assessments, care planning, and referral coordination for individuals seeking access to mental health services. This role focuses on engagement, stabilization, and linkage to appropriate Health Choices-funded services without duplicating reimbursable treatment.

Requirements

• Master’s degree in Social Work
• Active Pennsylvania LCSW
• 3+ years of behavioral health experience

Responsibilities

• Conduct individual and family mental health assessments
• Develop individualized, trauma-informed care plans
• Provide short-term supportive counselling and engagement
• Coordinate referrals to mental health, housing, and crisis services
• Work with hospitals, schools, juvenile justice, and child welfare
• Maintain documentation in CIPS
• Advocate for client rights, equity, and language access
