About The Position

Central Neighborhood Health Foundation is a Federally Qualified Healthcare Center (FQHC) committed to the Triple Aim as described by the Institute for Healthcare Improvement. Improving the US health care system requires simultaneous pursuit of three aims: 1) improving the experience of care, 2) improving the health of populations, and 3) reducing per capita costs of health care. Preconditions for this include the enrollment of an identified population, a commitment to universality for its members, and the existence of an organization (an "integrator") that accepts responsibility for all three aims for that population. Responsible for providing initial evaluations and individual, group, couples, and/or family therapy at the assigned CNHF clinic. In addition, therapists will provide behavioral health consultation services to the Primary Care Teams within Pediatric and Adult clinics.

Requirements

  • Master’s Degree in social work from an accredited college
  • Current licensure as a clinical social worker
  • Excellent verbal and written communication skills.
  • Excellent interpersonal, customer service, and conflict resolution skills.
  • Excellent organizational skills and attention to detail.
  • Excellent time management skills.
  • Strong analytical and problem-solving skills.
  • Must be effective and efficient in delivering culturally competent care to patients.
  • Must have the ability to exercise sound judgment.
  • Must be well organized, a self-starter, and possess excellent telephone communication skills.

Responsibilities

  • Performs client orientation into the CPSP program: services provided, OB staff, program adherence, and qualifications.
  • Complete all required documentation forms for the CPSP program.
  • Conduct initial mental health evaluations and develop realistic treatment plans.
  • Assessing clients using standardized assessment/screening tools, measures, etc.
  • Provide brief, solution-focused therapy.
  • Provide information and education, and referral services for identified issues.
  • Provide care coordination.
  • Conduct crisis assessments/triage with appropriate interventions.
  • Provision of other services related to wellness
  • Accurate and timely documentation of services provided in accordance with requirements
  • Perform other related activities designed to provide integrated health care services
  • Provide individual psychotherapy with children, adults, and families
  • Provide appropriate and timely referral to a higher level of care as required
  • Performs other duties as assigned
