About The Position

A Licensed Clinical Social Worker (LCSW) at AIDS Healthcare Foundation (AHF) integrates the organizational mission with the delivery of care management services. This role focuses on providing preventive health services to avert disease, disability, and other health conditions or their progression, aiming to prolong life and promote physical and mental well-being. The LCSW is instrumental in improving member health outcomes and health equity, contributing to longer, healthier, and happier lives. They support members in various settings—home, community, or clinic—by promoting adherence and compliance to care and helping to identify socio-economic factors impacting overall health. The LCSW is expected to be a passionate advocate for AHF's initiatives, upholding the AHF Core Values and Mission, and adapting to a dynamic environment to ensure the best experience for clients and employees. Working within a collaborative organizational structure, the LCSW is part of a close-knit team providing complex care management and culturally and linguistically responsive support to members within the HIV/AIDS Special Needs population enrolled in a Medicare/Medi-Cal health plan. The role involves applying advanced clinical practice skills to support the Care Team by identifying, cataloging, and coordinating community resources, and acting as a liaison for members' psychosocial needs. The LCSW actively participates in interdisciplinary teams, contributing to assessment, data gathering, and care planning, and engages in relevant committees to enhance understanding of social determinants of health affecting members. They address psychosocial barriers related to chronic and infectious diseases, mental health, substance use disorders, preventive services, perinatal/reproductive health, environmental challenges, oral health, aging, injury, and domestic violence. The LCSW also mentors Community Health Workers, assists members in crisis, manages referrals, and oversees behavioral health transitions of care in collaboration with the Utilization Management Department and Care Team Managers. They lead behavioral health meetings, track referrals, update leadership on successful linkages, and ensure timely, accurate documentation consistent with organizational policies, accreditation standards, and regulatory requirements.

Requirements

  • Masters of Social Work from an accredited school.
  • CA Licensed Clinical Social Worker (LCSW).

Nice To Haves

  • At least two (2) years of social work experience in Managed Care.
  • Bilingual Spanish.

Responsibilities

  • Apply advanced clinical practice skills to support the Care Team by identifying, cataloging, and coordinating community resources.
  • Act as a liaison to address the psychosocial needs of members.
  • Actively participate as a member of the interdisciplinary team, contributing to assessment, data gathering, and care planning.
  • Participate in CMS, DHCS/DMHC, and other relevant committees to strengthen organizational understanding of the social determinants of health affecting the members served by the plan.
  • Ensure members are informed about and have access to their health plan and community benefits.
  • Address psychosocial barriers that impact the management, control, or prevention of chronic and infectious diseases; mental health conditions and substance use disorders; need for preventive services; perinatal and reproductive health needs; environmental and climate-related health challenges; oral health; aging; injury; and domestic or interpersonal violence.
  • Demonstrate leadership by serving as a mentor to Community Health Workers (CHWs).
  • Assist members in crisis.
  • Manage referrals.
  • Oversee behavioral health transitions of care in collaboration with the Utilization Management Department and Care Team Managers.
  • Lead behavioral health meetings.
  • Maintain tracking of all referrals.
  • Regularly update leadership on successful linkages to care.
  • Update care plans.
  • Ensure timely, accurate documentation consistent with organizational policies, accreditation standards, and regulatory requirements.
