About The Position

The Licensed Clinical Social Worker at KD supports the needs of patients served in the Outpatient Behavioral Health office through provision of clinical social work services, including screening and brief intervention, assessment, diagnosis, treatment planning and coordination, and monitoring of mental health needs. He/she will offers a mix of traditional outpatient counseling services with onsite behavioral health consultation services. He/she serves as a member of the care team, available to serve all patients receiving primary medical, oral or behavioral health care in the clinic. This will include, but is not limited to, the assessment and treatment of individuals, families, and groups, as well as crisis intervention and engaging with patients as they present for services. The LCSW will collaborate with other involved providers to achieve the best care for patients and will strive to promote whole person improvement of health outcomes.

Requirements

  • Licensed Clinical Social Worker.
  • Maintains unrestricted Kentucky LCSW license and meets ongoing licensure requirements.
  • Achieves and maintains Non-Violent Crisis Intervention Certification.

Responsibilities

  • Participate as an active member in the patient's outpatient care team.
  • Assist colleagues in managing behavioral health crises in the clinic.
  • Maintain a visible presence in the clinic when not engaged in therapy sessions to promote collaboration between disciplines.
  • Utilize EPIC and care team huddles to provide insight and direction related to behavioral health needs.
  • Utilize clinical interventions from a variety of theoretical bases to treat patient mental health or substance use conditions.
  • Assign appropriate diagnosis for a range of behavioral health conditions, including substance use disorders.
  • Engage patient in identification of specific treatment goals and interventions/supports needed to achieve these goals.
  • Utilize standardized screenings and assessments in accordance with organizational clinical practice guidelines and quality improvement initiatives.
  • Provide individual, family, couples and/or group therapy to patients when applicable.
  • Provide care management services to patients with behavioral health and substance use needs.
  • Ensure all records are adequately detailed, problem-oriented, timely, truthful, clear and accurate.
  • Complete clinical notations on the day that services are provided.
  • Participate in the MSW programs in student practicum rotations and supervision of KD CSW's seeking LCSW degrees.
  • Participate in staff meetings and quarterly provider meetings as requested.
  • Assist with other duties or in other departments as instructed by supervisor.
