Job Overview The Licensed Clinical Social Worker (LCSW), Senior and Comprehensive Care is responsible for providing a variety of care management, patient education, therapeutic interventions, and social support services within a PCP practice(s), a patient’s home, or a community setting, targeting physically, socially, and behaviorally complex patients. The LCSW understands the pathophysiology of chronic conditions and is comfortable conducting psychosocial assessments and providing education, escalating to Providers, PharmD, and pertinent interdisciplinary team members when appropriate. The LCSW requires techniques in motivational interviewing, cognitive behavioral therapy, patient engagement and empowerment, and coaching/teach-back methods for self-management. The LCSW uses standard assessments and evidence-based screenings and develops unique patient-centered care plans, collaborating with patients, their support systems, and community service programs. The LCSW is a systems thinker and is skilled at thinking outside of the box for solutions to complex problems impacting patients and their caregivers.

Requirements

  • Professional Licensure as a Licensed Clinical Social Worker by the California Behavioral Science Board.
  • Master's level degree required.
  • Minimum of one year’s experience working with adults, preferably seniors aged 55+ years old, and a minimum of one year of clinical experience.
  • Direct experience with short-term problem-solving treatment (MHIP), cognitive-behavioral therapy, crisis intervention, brief psychodynamic therapy, family therapy, group therapy, child therapy, substance abuse treatment, and behavior modification.
  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
