About The Position

This position is responsible for assisting patients and families with behavioral health and environmental resources to reach the maximum level of functioning during the current episode of care. The role includes conducting behavioral health psychosocial assessments and clinical interventions, including individual, group, and family therapy. In collaboration with the interdisciplinary treatment team, the position coordinates patient care and provides case management for assigned patients.

Requirements

  • Master's Degree in Social Work from an accredited graduate program.
  • 1 Year Experience in a behavioral health setting.
  • California BBS Licensed Clinical Social Worker (LCSW) - REQUIRED.
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - REQUIRED.

Nice To Haves

  • 3 Years Experience in a behavioral health setting.
  • Acute/psychiatric hospital experience, discharge planning/case management experience.

Responsibilities

  • Maintain ongoing communication with multi-disciplinary team members regarding patient status and follow-up.
  • Consult with team members in the development of treatment plans.
  • Attend treatment team and staff meetings, in-services, and case presentations as designated by unit/department/program.
  • Arrange and facilitate patient care conferences for difficult/high-risk psychosocial cases.
  • Identify bioethical dilemmas and initiate appropriate responses.
  • Testify at Certification Review and Reise hearings regarding patient's current mental health status as appropriate.
  • Consult, coordinate, and intervene in crisis situations while assisting staff in de-escalating volatile situations.
  • Utilize appropriate community resources on behalf of patients and families.
  • Develop an appropriate and timely discharge plan.
  • Educate patients and families of options for discharge based on patient's mental health/legal/insurance status.
  • Provide continuity of care referrals to patients and families for ongoing community services/support upon discharge.
  • Schedule follow-up appointments with psychiatric providers within 7 days of discharge.
  • Complete thorough safety planning with patients and families.
  • Provide professional role modeling and collaboration with interdisciplinary team.
  • Design, implement, and/or support evidence-based processes and therapeutic programs for patients and families.
  • Assess, evaluate, and document the psychosocial needs of the patient and family.
  • Perform appropriate patient assessments related to the clinical services provided.
  • Complete all documentation elements in a timely, complete, legible, concise, and accurate manner.
  • Demonstrate knowledge and safe use of therapeutic procedures and treatment modalities.
  • Report all mandatory protective concerns as required by law and Sharp HealthCare policies.
