About The Position

The Licensed Clinical Social Worker (LCSW), reporting to the Social Services Program Manager, is an integral member of the Ambulatory Care Management department. Co-located within primary care offices, the LCSW provides direct care management services to patients, including comprehensive face-to-face biopsychosocial assessments and collaboration with primary care physicians to support diagnosis and brief therapy treatment. The LCSW also addresses social determinants of health and assists with advance care planning. This role primarily serves adult and geriatric patients presenting with behavioral health needs in a primary care setting. Patients referred by their primary care providers are evaluated and treated by the LCSW, who works closely with Care Management team members and Primary Care Providers as part of the interdisciplinary team to ensure optimal outcomes. In every interaction, the LCSW demonstrates respect, empathy, and cultural sensitivity toward patients, families, visitors, and staff.

Requirements

  • MSW from an accredited school by the Council on Social Work Education.
  • Minimum of 2+ years clinical social work experience in mental health.
  • KY - Licensed Clinical Social Worker (LCSW).

Responsibilities

  • Provides clinical social work, focusing on assessment and collaboration with patient's primary care visit to determine diagnosis and treatment of emotional and mental illness.
  • Develops comprehensive psychosocial assessments examining current problems and relevant bio-psychosocial and environmental factors.
  • Initiates contact with patient/family within 1 business day from referral date.
  • Communicates appropriate information on chart in accordance with HIPAA guidelines.
  • Creates a positive therapeutic relationship with clients to correct dysfunction.
  • Provides a wide range of treatment modalities based on assessment, including short-term CBT, solution-focused therapy, motivational interviewing, and more.
  • Identifies social determinants impacting patient's treatment and refers patients for case management needs.
  • Develops, implements, and modifies treatment plans.
  • Evaluates progress on patient care plans and terminates treatment or refers to higher level of care when necessary.
  • Initiates contact with primary care patients discharged from the emergency room for further mental health needs.
  • Documents all visits and phone contacts accurately and in a timely manner in EPIC.
  • Initiates patient/family education and counseling to enhance problem solving and coping.
  • Acts as a liaison between patient/family and healthcare team members.
  • Serves as a consultant to the interdisciplinary team regarding psychosocial components.
  • Communicates effectively with co-workers and healthcare team to ensure continuity of care.
  • Maintains skills and knowledge appropriate to the medical social work role and participates in continuing education.
  • Assists in evaluating patient outcomes and financial and clinical results.
