About The Position
We are adding an LCSW to our integrated care team to provide brief therapy and support care coordination for adult patients. This role offers a strong balance of clinical work, collaboration, and practical patient impact.
Requirements
- MSW from an accredited program
- Active CA LCSW license (ASW accepted; supervision provided)
- Minimum 1 year of clinical experience
- Familiarity with DSM-5 and evidence-based practice
- Basic computer skills and EMR proficiency
Nice To Haves
- Community health experience
- Trauma-informed care training
- Spanish fluency is a plus
Responsibilities
- Conduct assessments and provide evidence-based brief interventions
- Screen for trauma and substance use concerns and coordinate next steps
- Partner with primary care providers on treatment planning and follow-up
- Provide referrals to specialty care and community resources
- Complete documentation in the EMR within required timeframes
- Participate in case reviews and interdisciplinary meetings
