About The Position

Navitas Healthcare, LLC is seeking Licensed Clinical Social Worker for an exciting Travel or Local job in Atlanta, GA.

Requirements

• Active G eorgia LCSW license (GA LCSW)
• Minimum 2 years of clinical experience in a hospital or acute care setting
• Current AHA BLS certification
• Strong communication, assessment, and documentation skills
• Ability to work collaboratively within a multidisciplinary healthcare team

Responsibilities

• Conduct psychosocial assessments for patients in inpatient and/or emergency settings
• Provide crisis intervention, counseling, and emotional support to patients and families
• Develop and implement individualized discharge and care coordination plans
• Collaborate with physicians, nurses, case managers, and other healthcare professionals
• Assist with discharge planning, including referrals to community resources and support services
• Support patients coping with chronic illness, trauma, or acute medical conditions
• Maintain accurate and timely clinical documentation in accordance with hospital policies
• Participate in care team meetings and contribute to treatment planning discussions
• Ensure compliance with hospital protocols, ethical standards, and regulatory requirement
