About The Position

Requirements

  • Master's degree in Social Work or related field.
  • Licensed Clinical Social Worker (LCSW) certification.
  • 2 years of recent relevant experience.
  • Clinical skills in biopsychosocial assessment and interventions.
  • Knowledge of transitions of care and community resources.
  • Ability to work effectively on a team and advocate for patients and families.
  • Knowledge of behavioral health and mental health assessment.
  • Demonstrated ability to communicate effectively, both verbally and in writing.
  • Proficient in using computer software for data entry and communication.

  • Knowledge of palliative care and end-of-life psychosocial needs.
  • Understanding of cultural and spiritual values in social work practice.
  • Familiarity with local county/state/federal resources for at-risk populations.
