Job Summary:# Provides psychosocial assessments, plans and implements various types of counseling, conducts and coordinates care planning for complex cases and participates in multidisciplinary team process. Victims of domestic violence, child and adult neglect and abuse, adoptions and other DSS issues shall be the Social Worker#s responsibility. The LCSW shall work closely with our team of professionals including physicians, nurses and staff to provide counseling, education and resource identification while providing quality services related to emotional and mental health. Must be knowledgeable with State and Federal laws as they pertain to adult and child abuse reporting and domestic violence. The LCSW will act as a resource consultant for providers, clinics, and navigators with individualized patient care planning. Must be compliant with State, Federal, Medical Group Practice guidelines. Utilizes technology and reports such as EHR, disease registries, PCP platforms and Managed Care, MSSP, Medicaid, and PCMH reports to perform job role.
