A Licensed Clinical Care Manager contributes to AHF by leveraging their mental health expertise and understanding of the Ryan White Program. This role involves collaborating with patients, physicians, and the Healthcare Center team to address needs identified in patient health and psychosocial Comprehensive Needs Assessments, and implementing Individualized Care Plans. Responsibilities also include facilitating and identifying service options, providing education on program eligibility, and understanding mental health or substance abuse patterns that impact client health outcomes, especially for Ryan White contracts. The manager screens clients for complex medical and psychosocial issues, establishes Comprehensive Assessments, Individualized Care Plans, follow-ups, referrals, and guides clients through health and support services. This work directly contributes to AHF’s success in meeting Ryan White contract deliverables and measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services. The role requires a passionate advocate who upholds AHF Core Values and Mission, adapting to a dynamic environment. AHF operates with a collaborative organizational structure where staff are accountable to multiple leaders. The Licensed Clinical Care Manager works within a close-knit team to improve healthcare outcomes, support medication adherence, address barriers to care, and provide brief interventions to help clients achieve viral suppression, better health outcomes, and an improved quality of life.

Requirements

  • Current and good standing Licensed Clinical Social Worker or Licensed Master Social Worker in the jurisdiction in which services are rendered.
  • A minimum of 1 year of Case Management experience.

  • Proficient Spanish speaker.

Responsibilities

  • Utilize mental health expertise and understanding of the Ryan White Program within their specific area.
  • Work collaboratively with patients, physicians, and the Healthcare Center team to address needs identified in the patient’s health and psychosocial Comprehensive Needs Assessment.
  • Implement an Individualized Care Plan based upon the needs identified.
  • Facilitate and identify service options for addressing a client’s needs.
  • Provide education and assistance so that eligibility for the program is understood.
  • Understand mental health conditions or substance abuse patterns that interfere with a client’s ability to improve health and social outcomes (for Ryan White contracts combining psychosocial needs and resources within Medical Care Management).
  • Screen clients for complex medical and psychosocial issues requiring medical case management services.
  • Bring the essence of the Ryan White Care Act to vulnerable populations.
  • Establish Comprehensive Assessments, Individualized Care Plans, follow-ups, referrals, and direction of clients through the entire spectrum of health and support services.
  • Directly contribute to AHF’s success in meeting the deliverables of the Ryan White contract.
  • Contribute to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.
  • Be a passionate advocate for AHF's top initiatives.
  • Ensure that the AHF Core Values and Mission stay top of mind.
  • Work as a part of a close-knit team to improve health care outcomes.
  • Support clients taking all their medications as prescribed.
  • Address barriers to care and treatment.
  • Provide brief interventions focused on a long-term achievable goal, working with the client towards viral suppression, better health outcomes, and quality of life.
