Licensed Certified Social Worker assists with patient management in an outpatient setting as part of a multidisciplinary care management team for Surgical Service Line (SUSL). Responsible for assessment of the psychological, financial, and other needs of the patients and families. Provides information, counseling, and referrals to other levels of care and appropriate resources for the patient.

Requirements

Master's degree in Social Work, Psychology, or a related field
LCSW License

Three (3) years of experience in Social Work.

Responsibilities

Works with an interdisciplinary team to facilitate patient care in an outpatient setting.
Provides direct case management and assists client services for patients and families.
Performs assessments of psychological, financial, and other patient and family needs.
Plans and implements care plan, screens new clients for stress, and provides emotional support.
Refers patients to counseling and other levels of care and resources
Explains community outreach programs available to patients and families.
Provides ongoing support for patients as needed.
Performs other duties as requested.
