About the position

The Licensed Case Manager plays a vital role in supporting individuals in affordable housing by providing direct case management, therapeutic services and crisis intervention to ensure residents maintain stable housing and improve quality of life. This role involves engaging formerly homeless and/or Medical eligible residents with high needs, including substance use, mental health issues, and complex medical conditions. This position collaborates with Resident Services and Property Management teams to enhance overall safety of residents and the building in which they work focusing on community cohesion to support housing stability and self-sufficiency.

Responsibilities

  • Collaborate with property and resident services teams to identify and engage residents in need of support.
  • Develop and implement Individualized service Plans addressing housing stability, medical care, and self-sufficiency.
  • Connect residents with community resources, entitlement programs, and treatment providers.
  • Monitor progress through regular check-ins and adjust plans to ensure goal achievement.
  • Provide individual counseling using evidence-based approaches including Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and Solution-Focused Therapy to address mental health substance use, and trauma recovery.
  • Facilitate psycho-educational and therapeutic support groups to improve resident mental health and coping strategies utilizing Dialectical Behavior therapy (DBT) skill training, Relapse Prevention, and Mindfulness-Based Interventions for stress management, emotional regulation, coping skills, grief support, addiction recovery, and self-esteem building.
  • Integrate harm reduction and person-centered approaches to support residents in achieving their self-identified goals, promoting autonomy, and enhancing engagement in the therapeutic process while maintaining a non-judgmental, trauma-informed framework.
  • Conduct mental health assessments, address risks, and respond promptly to crises.
  • Assist in resolving disputes between residents, property management, and/or neighbors to promote safety and housing stability.
  • Collaborate with internal and external providers for comprehensive care and safety.
  • Maintain accurate case files and ensure compliance with organizational and HIPAA standards.
  • Prepare required reports and participate in quality assurance efforts.
  • Report suspected child, dependent adult and elder abuse and neglect according to State Law.
  • Mentor resident services staff and support difficult resident situations.
  • Attend organizational and community meetings, as well as resident events.
  • Contribute to program development, evaluation, and implementation of policies.
  • Other duties as assigned.

Requirements

  • California LCSW, LMFT, or LPCC; ASW considered but clinical supervision unavailable at this time.
  • 3+ years in mental health, substance use, or social services; 5+ years experienced in a housing setting preferred.
  • Proficient in Microsoft Office, Salesforce.
  • Experience with HIMS preferred.
  • Strong communication and crisis management skills, and commitment to harm reduction.
  • Flexibility for occasional night/weekend work.
  • Valid CA driver's license and background clearance.
