About The Position

LCSW / LPCC / LMFT Location: Anderson County School District Cumberland Family Medical Center (CFMC) operates a Federally Qualified Health Center (FQHC) offering a wide range of health and mental health services at multiple sites throughout Kentucky. The LCSW/LPCC/LMFT provides comprehensive psychosocial assessments and behavioral health care. Services are designed to support and maximize the client’s ability to better direct their own health care experiences using assessment, intervention, education, and linkage to health and community resources. The LCSW/LPCC/LMFT encourages respect for individuality, independence, patient choice and collaborates on policies and systems that respect the cultural, religions, and ethnic differences among patients and families. Mental health services contribute to the medical team's understanding of patient beliefs, behaviors, and barriers regarding the client's illness.

Requirements

  • Master’s Degree/Licenses, Certifications, Registrations.
  • Current unrestricted license in the state in which practicing is required upon hire.
  • Full-time position, 5 days per week, 100% clinic-based, in person work required.
  • Work schedules are set at the discretion of CFMC and may be adjusted at any time based on the operational needs of the organization.

Responsibilities

  • Uses age appropriate assessment and intervention skills. Able to use a range of interventions, depending upon the needs of the patient/family.
  • Assists members in developing cost effective, appropriate health care plans through optimizing the member's personal resources as well as community resources and programs.
  • Promotes multi-disciplinary care that is patient-centered and considers all aspects of members' personal, psychological, economic, and cultural needs.
  • Maintains thorough knowledge of CFMC services/benefits, public/private community resources, as well as federal/state laws which effect health care planning.
  • Completes required documentation of services and plan of care per policy.
  • Participates in program development, orientation, and educational activities which further members' capacity to better direct their own health care experience.
  • Other duties as assigned.
