The Behavioral Health Professional (BHP) is an integral member of the primary care team, providing brief, evidence -based, solution -focused behavioral health services. The BHP supports patients with mental health, substance use, chronic conditions, and life stressors through short, focused appointments (15–30 minutes), delivered in -person, via telehealth, or by phone. The role emphasizes functional restoration, patient empowerment, and collaboration with the primary care team to ensure comprehensive, coordinated care.

  • Conduct initial and ongoing assessments, including diagnostic evaluation, risk assessment, and level -of -care determination.
  • Provide brief therapeutic interventions, crisis management, and patient/family education.
  • Maintain a minimum of 4 patient contacts per day (after 6 months).
  • Collaborate with primary care providers to develop and implement care plans.
  • Monitor patient progress, using standardized screening tools, and communicate updates to the care team.
  • Complete professional billing accurately and timely for behavioral health services.
  • Assess and manage suicidal, homicidal, or abuse risk; coordinate with crisis teams or authorities as needed.
  • Support population health outreach and proactive patient engagement.
  • Provide consultation and training to primary care staff on behavioral health and substance use topics.
  • Master’s degree in Social Work or Counseling.
  • 1–3 years clinical experience, preferably in primary care.
  • Montana LCSW or LCPC license required; BLS required; credentialing with payors within 3 months of hire.
