How you’ll make an impact in this role Lead Personalized Recovery Strategies: Conduct deep-dive assessments to establish meaningful therapy goals that empower patients to take ownership of their mental health journey. Facilitate Transformative Therapy: Deliver individual and group sessions that provide patients with the coping tools and emotional breakthroughs necessary for long-term stability. Synchronize Collaborative Care: Act as the central link between families, physicians, and community providers to ensure a seamless, supportive experience from the first day of treatment through discharge. Evaluate and Adapt Interventions: Continuously monitor patient progress to ensure every therapeutic action is effective, making real-time adjustments to optimize recovery outcomes. Bridge the Transition to Home: Connect patients and families with vital community resources and activities, ensuring they have a "warm handoff" and a clear map for success after leaving our care. What minimum requirements you’ll need Licensure / Certification / Registration: One or more of the following required: Licensed Associate Counselor specializing in Mental Health obtained prior to hire date or job transfer date. Will accept any Specialty. Licensed Associate Counselor specializing in Clinical Addiction credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained prior to hire date or job transfer date. Licensed Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained within 12 Months (1 year) of hire date or job transfer date. Or eligible by hire date. Licensed Associate Therapist specializing in Marriage & Family credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained within 48 Months (4 years) of hire date or job transfer date. BLS Provider preferred. American Heart Association or American Red Cross accepted. Education: Bachelor's degree in Social Work required.
